Received: 23-05-11 Accepted: 01-06-11 Abstract This is a clinical case of a 71-year-old man with an obstructive lesion of the hepatic hilum, the clinical, imaging studies and the lab tests suggest a Klatskin tumor, which was managed by percutaneous biliary drainage to relieve the jaundice. In this review we consider the pathogenetic factors of these tumors, a new anatomic classifi cation and the various alternative imaging studies, emphasizing the Bismuth-Corlette classifi cation. Confront palliative alternatives, such as different types of endoscopic stents or percutaneous approach, drainage extension (uni or bilateral). Finally, we review the surgical aspects and possibilities of chemotherapy and radiotherapy in these tumors.

Long-term survival after intraluminal brachytherapy for inoperable hilar cholangiocarcinoma: a case report.
Surgical resection with a tumor-free margin is the only curative treatment for hilar cholangiocarcinoma (Klatskin tumor). However, over half of the patients present late with unresectable tumors. Radiotherapy using external beam irradiation or intraluminal brachytherapy (ILBT) has been used to treat unresectable hilar cholangiocarcinoma with satisfactory outcome. We reported a patient with unre...

متن کاملEndoscopic Biliary Drainage of Hilar Cholangiocarcinoma
Cholangiocarcinoma is an uncommon malignancy which arises from biliary epithelial cells, anywhere along intraand extrahepatic biliary tract and can be divided into three groups by involved anatomy: intrahepatic, perihilar, and distal extrahepatic cholangiocarcinoma. Cure can only be achieved by surgical resection. Because of initial diagnosis as advanced disease and recurrence despite surgery a...
متن کاملUnresectable hilar cholangiocarcinoma: multimodality approach with percutaneous treatment associated with radiotherapy and chemotherapy.
Klatskin-type cholangiocarcinoma is a rare tumor, bearing a very poor prognosis: at diagnosis, most patients can only undergo palliation. Evaluation of outcome, mean survival and quality of life was performed in patients with unresectable hilar cholangiocarcinoma treated with multimodality approach in comparison with surgical palliation, biliary stenting or brachytherapy alone. Twenty-six patie...
متن کاملOptimal Endoscopic Palliation of Advanced Hilar Cholangiocarcinoma with Metal Stents
Endoscopic biliary stenting is a well-established palliative procedure for treating obstructive jaundice in patients with unresectable hilar cholangiocarcinoma. Plastic tube stents (PS) have been widely used for the procedure, but self-expandable metallic stents (SEMSs) are becoming increasingly popular because of the associated longer patency. However, there are several unanswered questions re...
